 Caution FormUse this single Caution Summary Report Form to list each caution (i.e. yellow card you display) to a player or substitute during a game.

If you caution a player twice, you must submit a Dismissal Form that contains a brief description of both cautionable offences.

Only players and substitutes can be cautioned. If you have to "warn" a team official about his or her conduct or the conduct of that team's spectators, you may have to submit a Special Incident Report Form depending on the circumstances.

 Dismissal Form 

Use a separate Dismissal Form for every player or substitute you send off (i.e. show a red card). In addition to providing the details about the game and the player, you must also describe the incident.

You should describe the incident so that someone who was not at the game will understand what happened.

If you send off two or more players as part of the same incident (e.g. a fight) you must submit a form for every player you dismiss.

Only players and substitutes can be sent off. If you have to expel a team official from the field, you must submit a Special Incident Report Form.

 Special Incident Form 

Use the Special Incident Report Form whenever you have to suspend a game (i.e. delay the game for an extended period of time), terminate a game, warn or expel a team official, or report some other serious incident.

A copy of this form must also be sent to the District Association (for local leagues) or the O.S.A. for regional leagues. Don't let a league "talk you out of" sending copies of these reports to the proper authorities.

You should contact any membre of the Branch Executive for assistance in submitting this form if there was a serious incident in your game.

 Assessment Request Form  

Use the Assessment Request Form to request a formal assessment for upgrading and maintainence purposes. Print the form, write on it your future game appointments, and submit it to:

  • Your D.R.C. if you are a Class 3 referee.
  • The Ontario Soccer Association if you are Class 2 or above
